is one of the most important things you can do for your health.
Step 3: REFER
to SOSWorks providers complete the Tobacco Treatment Referral
Form and fax to SOSWorks. Call SOS Quitline directly @ 1-866-669-7848
SOSWorks
proactively refers your patient to the evidence-based tobacco treatment
programs
in Arkansas.
- The Arkansas
Tobacco Cessation Network (ATCN) provides on-site treatment at
numerous locations around the state.
- The Arkansas
SOS Quitline provides treatment over the telephone.
SOSWorks
follows up after 7 days and again after 3 months, provides patients
with motivational counseling, and faxes you follow-up reports for
your patients chart. Treatment is provided at no cost to the
patient. Patients may qualify for medications at low or no cost.
The evidence-based treatment your patient will receive involves:
